Thoracic Aortic Aneurysm

Definition

  • Pathologic dilatation of a segment of the thoracic aorta
    • True aneurysm: involves all 3 layers of the vessel wall
    • Pseudoaneurysm
      • Intimal and medial layers of the vessel wall are disrupted.
      • Dilatation is lined by adventitia only and sometimes by perivascular clot.
    • Fusiform aneurysm
      • Affects entire circumference of a segment of the vessel
      • Makes a diffusely dilated lesion
    • Saccular aneurysm
      • Involves only a portion of the circumference
      • Results in outpouching of the vessel wall
    • Thoracoabdominal aortic aneurysm
      • Aneurysm of the descending thoracic aorta
      • Usually contiguous with infradiaphragmatic aneurysms
